2134 Issuance of contributory income certificate (УП 2)
The insurance income on which contributions have been paid or are payable to employees whose service or employment relationship has already been terminated shall be ascertained, in addition to the data provided from time to time by the insurer to the National Revenue Agency or by an insurance booklet, by a document on an approved model (УП 2) issued by the insurer.

According to Article 29, paragraph 4 of the Rules of Procedure of the Ministry of Innovation and Growth, the working hours of the administrative units that provide administrative services to citizens are from 9 am to 5.30 pm. The office hours of the Administrative Service Centre for Citizens shall be from 9 am to 5.30 pm on weekdays without interruption. Telephones for contacting the Administrative Service Centre are at the cost of a city call, according to the dial-in rate.
